Update an Encounter Note

Caution! Other users are not blocked from opening a note while you are attempting to edit it. You will not be able to save your changes if another user opens the same note and saves their changes before you have a chance to save yours. If this happens, you will be prompted to do one of the following:

Return to your note where you can manually copy your changes and then exit out of edit mode. After manually copying your changes, you can use that is available when editing a note to add any updates made by other users. You can then add your changes to the note.

Close without saving your changes. In this case, the note refreshes and displays the most recently saved version (including any changes that may have occurred while you had the note open for editing).

If another user finalizes the note while you still have it open, you will be prompted to do one of the following:

Return to your note where you can manually copy your changes and then close the note.

Create an addendum where you can re-type or manually copy your changes.

1 In the Notes in Progress section of the patient summary, click the link for the note that you want to update.

You can only make changes to notes that are not yet finalized or marked erroneous. To make changes or to add an associate user to a finalized note, you must add an addendum. For more information, see Add an Addendum.

2 View existing note information that has been added and saved.

3 To make changes to the Date of Service that appears at the top of the note, click then click the correct date.

4 To make changes to the Associated User list, click the user to associate with the encounter note.

5 Select or clear the Urgent/Emergent Situation check box.

6 To make changes to the visit type, make another selection from the list.

7 If necessary, change the ICD code set.

Note: Any ICD codes you have already entered into the encounter note remain as is. Only newly added codes will reflect the selected code set. Additionally, the code set for billing details will not change.

8 To make change to the Responsible Provider, click the provider from the list who is responsible for the patient’s care during the visit.

9 To make changes to the location, make another selection from the list.

10 To change the template assigned to the note, click the Select Template link and make another selection.

11 Continue documenting the patient encounter with any applicable information.
