You can ensure that a patient’s list of medications is accurate by reconciling all currently active medications as well as those that appear in the patient’s claims history, pharmacy history, and inactive medications lists. Additionally, if a new electronic referral or transition of care document is available for the patient, medications contained within those documents are available for reconciliation.
When reconciling medications for a patient, you can do the following:
• View a complete list of a patient’s medications and verify the information is accurate
• Remove a medication from the patient’s list of active medications and specify an inactivation reason
• Add a medication from the patient’s claims history, pharmacy history, and all inactive medications
• Add and then edit a medication listed in an external clinical document (received in CCDA format)
• Add a reported medication and indicate that it is active.
• Edit a reported or administered medication.
Active medications appear in the Care360 Active Medications list until they are manually deactivated. Inactive medications appears in the patient’s Care360 Inactive Meds list.
