Meaningful Use CQM Reports

To qualify for government incentive payments, providers should report on specific quality measures that are designed to monitor the quality of patient care. Each measure calculates a percentage of compliance for a certain group of a provider’s patient population, such as patients over 50 years old, or patients 2 and under. All patients included on the report for a specific provider must have that provider designated as the primary provider. Care360 EHR calculates the measure percentage based upon the clinical information available in Care360 EHR or HD (for iPad®) that is relevant to the measure being monitored for the particular patient population. This section describes each available quality measure report and the criteria for the numerator and denominator that is used in arriving at the report result percentage.

Two different factors that may affect patient population calculations include:

Patients not included in the total population. Some of the quality measures reports do not count all patients in the population for certain situations. For example, when a rule measures all patients 18 years and over, patients 17 and younger are not included in the report. See the individual quality measure report description to determine if this situation applies to a particular measure.

Patient exclusions and exceptions. Some of the quality measures reports allow for patient exclusions and/or exceptions in situations where it is not appropriate to count a patient as part of the initial patient population being used for calculating the measure percentage. Exclusions and Exceptions display as separate counts next to the report results, if applicable.
