Viewing Patient Population Alerts

Only Care360 EHR organizations that have Clinical Decision Support are configured for automatic analysis of clinical care for the entire patient population within the organization. Your organization can identify which medical conditions to monitor. For each condition, the system will analyze the patients that have been diagnosed with the specific condition, along with relevant clinical information, to determine if any gaps in care exist based upon the clinical guidelines for that condition. This analysis allows you to identify the segments of your patient population that require attention or additional care.

Note: The patient compliance information that is available on the Quick View page can be filtered by the provider that is designated as the patient’s primary provider in the system, or can display all of the patients in the organization (including those patients who have not designated a primary provider). If you are a provider, the compliance percentages on the page are initially filtered to only show the patients for whom you are the primary provider.

When gaps in care are identified, you are notified on the Quick View page with the following compliance indicator, when applicable:

Active Lab Recommendations. The number of recommendations for patients in your organization who are identified as having lab result values which suggest further lab testing may be necessary. Counts include the total number of active and dismissed recommendations, and the number of patients with recommendations. For more information on viewing lab recommendation, see Viewing Recommendations.

Note: If a patient has a priority lab that needs to be reordered for more than one guideline, the patient is only counted once, but each guideline is counted individually.

Compliance by condition indicators. The compliance by condition indicators display percentages of the overall patient population who are in compliance for individual conditions being monitored for your organization or the percentage of patients that meet the quality measure requirements (associated with Meaningful Use reports). Notes:  

Patient population alerts that display on the Quick View page may not reflect recent updates to a patient’s clinical information. However, results are expected to be no more than 24 hours old.

Information in encounter notes that have been marked erroneous will be excluded from all clinical decision reporting.
